White: Artic Blue mixed with Art White, highlighted with Art White. Shaded with Drakenhof Nightshade.
Bone and parchment: Pink Flesh then Off White, highlighted with Art White and shaded with Targor Rageshade.
Red: Off White followed by Berserker Bloodshade.
Green: Sap Green highlighted with Primary Yellow. Dot of Art White on the lens

This guy is part number 70155/1. He was released in July 1995 (announced in White Dwarf 187) and is a Jes Goodwin sculpt.
He was a 1994 sculpt. His right arm was the usual storm bolter 0441/4 and the left was the Crozius Arcanum 70155/2.
