This metal Rogue Trader miniature was one of the original Mechanicus sculpts by Bob Olley. I can’t make out the year on the back of the tab. He goes by 401101 or 051301.
He can be seen in White Dwarf #111:

…and again in White Dwarf #112, painted by Paul Benson:

Pete Knifton has an illustration in White Dwarf #110 which looks remarkably like him, though he has swapped which way he’s holding the axe:

The front of the tab says “ADEPTUS MECHANICUS”, although this example has had it scrubbed off for the most part. The back says “GW 1988”.
